Liability Waiver AND HOLD HARMLESS AGREEMENT

By signing this agreement, the animal owner agrees to indemnify and hold Kissable K9 Care Inc, and all employees and agents of Kissable K9 Care Inc. harmless from and against all claims, demands or causes of action, liability, loss, damages, or expenses, including attorneys’ fees and costs, arising out of or in the course of any animal teeth cleaning activities.

This indemnity and hold harmless agreement shall include any claim, demand, or cause of action against all companies or any of its employees, officers, or shareholders for negligence and/or strict liability. We may use homeopathic remedies to calm your pet such as lavender.

To release and hold (clinic host selected in the form), its principals, officers, directors, agents and/or employees harmless from and against any claims, liabilities, demands, actions or causes of action for any injuries, harm, loss, inconvenience, or damages howsoever suffered arising from the services provided.

Every effort will be made to clean and remove all the tartar from your pet’s teeth, within the limits as prescribed by statute and case law for a person who is not licensed to practice veterinary medicine. It is important to note that your pet will be awake and depending on how much they wiggle it may be impossible to remove all tartar above the gum line from their teeth. We strive to be on time for your appointment. We have no control over how long the dog in front of yours takes, so please be patient. Any dog with questionable health, or with possible or suspected problems beyond the scope of this cleaning will be referred to see a veterinarian. NOTE: We are not veterinarians, and this procedure should not be considered a veterinary cleaning but a cosmetic cleaning only.
